Full-Time Intake & Services Coordinator

Job Description

Position Summary:

The Intake & Services Coordinator at the Center for Early Relationship Support at JF&CS serves an important role on our team as the first point of contact for all inquiries seeking or referring clients to our direct services. The Coordinator will be a warm and informed first contact to listen to prospective clients, assess their presenting needs, and connect them with a service provider in a warm handoff and follow up. The Coordinator will be integral across the division and interact with a wide range of clinicians, staff, referring professionals, and parents of young children, including those seeking support for concerns with the transition to parenthood, parental mental health, trauma, substance use disorder (SUD), infant health/prematurity, and/or challenges in the parent-child relationship.

This is a full-time position of 36.25 hours/week with flexible schedule options.


  • Coordinate with referring providers to ensure documentation, client follow-up, and referral follow-up are completed and recorded in a timely manner
  • Conduct initial interview of prospective clients/participants to determine the priorities for services based on the presenting needs, gathering preliminary demographic information for funding and grant purposes
  • Connect with CERS program leads to maintain an ongoing sense of program capacities and priorities
  • Connect clients/participants with appropriate services within CERS and JF&CS with a warm handoff to the designated staff member/clinician
  • Connect clients not eligible for CERS services to other community resources
  • Use client management system to track and document all aspects of client-interfacing work
  • Maintain and update intake documents and information specific to programs and funding requirements
  • Demonstrate and operationalize awareness of the impact of diversity and systemic inequality in interactions and relationships with clients, colleagues, and systems
  • Other duties as assigned

Educational/Experience Requirements:

  • Combination of education and experience that reflects typical skills gained from a Bachelor’s degree in Social Work or related field and direct experience working in Human Services
  • Experience assessing clients’ needs, preferences, and vulnerabilities
  • Experience triaging and seeking support for clinical dilemmas/crises when they arise
  • Experience administering and managing client-centered paperwork to ensure a balance of clinically appropriate and administratively thorough data collection and documentation
  • Understanding of services related to children, families, and communities
  • Experience contributing positively to a large integrated team/division
  • Bilingual Spanish speaking preferred
  • Excellent listener and interviewer

  • Comfortable working with large number and variety of people
  • Knowledge of Microsoft Office computer programs (Excel, Word, Outlook, PowerPoint) and willingness to learn and use technology for record-keeping and communication.
