Job Description

Duty hours: 9 AM – 5 PM EST (Part-time: 20 hours per week) 

 

We’re looking for a compassionate Care Manager to develop individualized patient care plans. Care Managers are primarily focused on overseeing and improving client and family care and health outcomes.  

The Care Manager will be responsible for the assessment, reassessment and coordination of care and services, including ongoing monitoring of appropriate and effective person-centered care plan, member education.  

He or she will regularly communicate with the people involved in the client circles i.e.: PCP, School and education workers and other providers and integrate the member, caregiver and other provider feedback into the assessment and planning.  

To ensure success, you will guide patients through the healthcare system and cultivate relationships with healthcare providers. Top candidates can multitask in a fast-paced environment and negotiate effectively.
