Full-Time Aging Life Care Manager

Position Summary:

Join Your Elder Experts our well-established care management team, one of the largest aging life care management practices in the Greater Boston Area.

This is an opportunity to work independently while also learning from and contributing to a team of highly skilled professionals.  Using creativity and interpersonal skills within a clinical framework we support our clients as they navigate the challenges and transitions of aging through maximizing choice, dignity, and engagement.

We offer one-time consultations, crisis management, or ongoing day-to-day care management.  You would be working in a variety of settings including directly in clients’ homes, medical settings as well as virtually.

This position offers a hybrid model of employment where some team meetings and other activities will require you to be in-person at our Waltham office while care management responsibilities would be in the field.

We provide both individual and peer group supervision.  This is a full-time position with benefits.  We do not provide round-the-clock support to our clients; however, our clients’ needs do not always fit into regular business hours and therefore require us to be flexible.  The team provides backup support for one another as needed and coverage for days off.

During your workday, you will:

  • Evaluate and assess the strengths and needs of older adults and adults living with chronic illnesses
  • Educate and coach clients and their families/care partners about options and resources
  • Create and manage client centered plans of care as well as being at the ready when plans need to be modified.
  • Provide emotional support and help facilitate client centered family/care team relationships
  • Develop relationships with community service providers and professionals in the field of aging
  • Document all work in electronic records
  • Participate in staff meetings, supervision, and training

What can you bring to this position?

  • Keen understanding of and experience with cognitive, physical, mental, and emotional capacities of older adults
  • Flexibility and initiative to manage crisis and on-going care management
  • Master’s Degree in social work, nursing or other allied health care professions preferred. Or a bachelor’s degree and 5+ years of experience in a similar position.
  • Knowledge of community resources, entitlements, and health insurance

You will need a car and a safe driving record!  Andthe ability to effectively use Microsoft Office Suite.
